Savvy Health Eligibility (“S.H.E.”, “the extension”) is an internal workflow tool for authorized staff of healthcare organizations that subscribe to Savvy Wound Report (“SWR”). This policy explains what the extension accesses, why, and how the information is handled. It applies only to the Chrome extension; the SWR web application and mobile app are covered by the separate Savvy Wound Report privacy policy.

In short: The extension helps authorized staff read Medicare eligibility information they have looked up in the Medicare portal and send the reviewed values into their own organization’s SWR patient record. It does not sell, share, or transmit data to us or to any third party for any other purpose.

1. Single Purpose

The extension has one purpose: to capture Medicare eligibility information (Part B status, Medicare type, and Skilled Nursing Facility admission/benefit details) that a staff member looks up in the Medicare portal, and to send those reviewed values into the staff member’s own SWR patient chart — eliminating manual re-typing.

All patient and eligibility data flows only between the user’s browser, the Medicare portal the user is already authorized to use, and the user’s own SWR system at swrchart.com. The extension performs no analytics, advertising, tracking, or profiling.

5. Data Storage & Security

Provider settings and the remembered email are stored locally in the user’s browser and never sent to us. Patient and eligibility data are transmitted over HTTPS to the organization’s own SWR system and stored there under that system’s existing security controls; they are not retained by the extension. No method of electronic transmission or storage can be guaranteed 100% secure, but reasonable technical and organizational safeguards are applied.

6. Authorized Use Only

This extension is intended solely for authorized workforce members of organizations that subscribe to Savvy Wound Report, acting within their permitted role and applicable law (including HIPAA) when accessing patient information they are entitled to access.

7. Data Retention

The extension does not maintain its own database. Locally stored settings persist until the user clears them or removes the extension. Patient/eligibility data written to SWR are retained by the SWR system per that system’s policy.

8. Changes to This Policy

We may update this policy from time to time; the “Last updated” date above will change accordingly. Continued use of the extension after an update constitutes acceptance of the revised policy.
